We appreciate you sharing your thoughts on the new Community-Police Complaint Mediation Program from the Citizens Police Oversight Commission (CPOC). 
  •      Your answers are anonymous (unless you voluntarily provide your contact information for a possible follow-up interview on Question 12 -- and if you do NOT want to provide this information simply enter "X" in the open spaces provided in Question 12).

This information will help CPOC, an independent commission that was created following the George Floyd murders in May 2020, to address community and police relationships in Philadelphia.
