Dear Participant,

I am a School Psychologist with Cypress-Fairbanks ISD conducting a study on perceptions of effectiveness related to school performance for parents of student with an Emotional Disturbance. Your help is needed to provide feedback on the presentation entitled, “Breaking Down ED: Helping Your Child Achieve School Success.” To assist in this study, we ask that you complete some questions to give a little background about yourself and how effective you feel about helping your child’s school performance. Completion of these questions should take no more than 10 minutes. You will then watch a video presentation about Emotional Disturbance and various interventions to assist your child emotionally and academically. Following the video you will again be asked several questions about your comfort level in assisting your child’s school performance. This will help us to determine the effectiveness of the presentation. I cannot guarantee, however, that you will receive any benefits from participating in this study.

Completion of this survey indicates your informed consent to participate. Completion of the survey is voluntary and participation in the study is voluntary. If you decide to participate, you are free to withdraw your consent and to stop your participation at any time without penalty or loss of benefits to which you are allowed. Your participation in this project is anonymous. Confidentiality will be maintained to the extent allowed by law. Risks of participation in this study are not greater, considering probability and magnitude, than those ordinarily encountered in daily life. The results of this study will be used to improve the presentation and may be published in professional and/or scientific journals. It may also be used for educational purposes or for professional presentations.
