This survey includes only student fatalities that occur in the loading and unloading areas of the school bus that were reported to the appropriate state agency. The bus loading and unloading area is defined as “any location the school bus stops to load or unload student passengers.” Fatalities include those that occurred when the student was going to or from school and when boarding or exiting the school bus:
1. In the loading and unloading area
2. In the vicinity of the loading and unloading area or
3. Crossing the road, street or highway

No other fatalities are to be included. If you are uncertain about a particular incident, report the incident and include as much information as possible in the synopsis section of the survey in order to record it properly.
Please answer the following three questions. If you do not have any fatalities to report the survey will automatically direct you to the final page for submission. If you have fatalities to report, the survey will automatically direct you to a data collection page for each fatality.
