You must be registered to attend this event. Preference will be given to students who have a disability.

1.I am willing to bring my students (who are physically able to attend) to the "How Tanner Has Become the Person He Is Today" event at OCtech on April 21, 2026.
2.This event is for students in the tenth through twelfth grades. Please enter the number of students you have in your class who will be attending the event. Each school is allowed 20 students per school.(Required.)
3.Building R Auditorium has two full rows of seats that can accommodate students in wheelchairs. One row is located on the first row of the Auditorium (in front of the stage) and the other is located off of the access ramp when you enter the Auditorium on the left side. If you choose to bring a student or students that requires wheelchair access, please indicate the number below.(Required.)
4.The other seats in Building R require individuals to have good mobility due to the steps and seating. Please choose individuals that have good mobility and are able to climb up and down the stairs.(Required.)
5.As a Special Education Teacher or SPED Teacher, I will choose students who will benefit from this event. I hope they will receive motivation as to what they could accomplish one day, with the appropriate resources available to them. It is my hope as your Regional Career Specialist; this will enable your students to realize the resources available to them for them to utilize.
6.My students will be dressed appropriately for the event, which entails the following: No hoodies are permitted, no cell phones, no Air Pods, no pants or jeans with holes in them, hats are not allowed in the building. If your student wears one, they will be asked to remove it. All of these rules will be strictly enforced.
7.I will encourage my students to behave in a respectful manner. They will give the speakers their undivided attention throughout the program. The speakers have graciously volunteered their time for this program.
8.My school/Career Center will provide transportation to and from OCtech. I will email you a map of where your bus driver should park the bus/minivan.

11.I agree to have a designated alternate chaperone, that has been approved by the Principal, just in case I become sick, have a sick child, or family emergency. Please list their name and information below.(Required.)
12.The alternate person will have all of the information to attend the event. My principal is aware of this and has approved this for the event.
13.I agree that if an unforeseen situation happens and my students cannot attend, I will contact Mrs. Jean Rickenbaker at Rickenbakerjh@octech.edu or via phone at 803-535-1232. Perhaps another school/class may be able to utilize our space.(Required.)
