You are a parent carer of a child or young person with Special Educational Needs and/or Disabilities (SEND) aged between 0-25 years of age

Live in Central Bedfordshire

You are a SNAP PCF member.

Please upload only the information needed to confirm your child's SEND or disability. You may redact information that is not relevant to confirming eligibility, including financial information, detailed medical information and information about other family members.
Examples of acceptable evidence include an EHCP, DLA/PIP award letter, NHS or health professional letter, or appropriate school/SENCO evidence such as a SEN Support Plan.
Please make sure your child's name is visible on the evidence.

Your evidence and privacy
The evidence you provide may contain information about your child's health, disability or SEND and may therefore constitute special category personal data under UK GDPR.
SNAP PCF will use this information only to verify eligibility for a Max Card and will restrict access to authorised individuals who need to carry out this check.
We will collect and retain only the information necessary for this purpose. Where possible, unnecessary information should be redacted before submission.
Supporting documents will not normally be retained once eligibility has been verified. Where a record of eligibility is sufficient, SNAP PCF will record that eligibility has been confirmed rather than retaining the full document.
