The UAW National Education Department works hard to provide you with tools and resources for use in your local union's education program.
 
With the approval of your local union leadership,
you're able to obtain certain curriculum kits for use in your local union, 
which is the purpose of this form.

Please have your Local Union President request support
through your Servicing Representative.
