Purpose of this Assessment

The Columbiana County Mental Health and Recovery Services Board is conducting an assessment to determine strengths and areas for improvement in its goal of establishing a “Recovery-Oriented System of Care”. In a Recovery-Oriented System of Care, MHRS Boards take the lead in working with the community at large to accomplish the following:

- Build a community infrastructure that supports substance abuse prevention and mental health promotion.

- Provide effective treatments for mental illnesses and addictions.

- Partner with the community to establish environments and supports to help people “regain their lives” – by maximizing their independence and by giving them opportunities to contribute to their communities.

Recovery-Oriented Systems of Care are well understood by the community, and are easy to navigate.  

We are asking a wide variety of people in Columbiana County to complete this survey. Please indicate to the degree to which you think each survey item reflects the activities, values, and practices of the Columbiana County community mental health and substance use prevention, treatment, and recovery system.  

The survey will take about fifteen minutes. No one who receives this survey will be knowledgeable on all the items, so please respond DK (don’t know) as appropriate. Rely on both your personal and professional experiences when responding. Your responses will remain confidential. 

Once you begin the survey, do not close out of the questionnaire if you are unable to complete it, as your responses will not be saved. Please minimize the survey and complete at a later opportunity.

Once the surveys are returned and the results analyzed, the MHRS Board will determine follow up actions. If you would like the results of the assessment and the MHRS Board’s follow up plan, or if you are willing to be contacted for follow up information, there is a place at the end of the survey for you to enter your contact information.
