It is understood that this authorization is given in advance of any specific diagnosis, treatment, or hospital care being required by and is given to provide authority and power on the part of our aforesaid agent(s), especially in case of emergency, to give specific consent to any such diagnosis, treatment, or hospital care which the aforementioned physician in the exercise of his or her best judgment may deem advisable.

I further consent that these may be used by such other parties to whom Watsonville First UMC shall give permission. This consent shall include any or all remarks/contributions/ performances that I/we may make or give in connection with my/our appearance/participation.

I acknowledge that Watsonville First UMC has full ownership rights to the video/audio/photographs and that its proceedings, including my appearance, may be transmitted or otherwise exhibited in any medium, without further authorization from me.

I represent that I am the parent/guardian of _________________, who is a minor, and that I hereby authorize, approve, and agree to the consent as stated above, and I agree that we are bound by it.
