The purpose of this survey is to identify your level of satisfaction with the Teen Pregnancy Prevention TOP. The survey is not a test - there are no right or wrong answers. All survey responses are confidential and anonymous – no one will know how respondents answer the items. Responses will be analyzed collectively with those of other respondents.
