The Sal Lucido Hypertension Hero Award Application

The National Hypertension Control Roundtable (Roundtable) is accepting nominations for our Sal Lucido Hypertension Hero Award. We are looking for innovative, collaborative programs or partnerships addressing inequities in hypertension control. You may nominate your own organization; nominations do not have to be members of the Roundtable.

Applications will be reviewed by an objective subcommittee of the RoundTable. The recipient will be announced at our annual summit in the Fall.

Nominations will be accepted through July 31st.
