You can report your own experience, or the experience of a family member or friend that you were supporting. Please only one survey per incident. We invite you to complete the survey on your first visit to the survey page. It only takes a few minutes! You can view the survey as many times as you want before completing it. However, after the first visit, you might see a pop-up message saying “This survey was modified by its creator.” Simply click DONE to continue and finish your survey.
