Client and Community Survey for Diversity, Equity, Inclusion & Belonging
at Family Transition Place (FTP)

Introduction

Family Transition Place (FTP) is committed to creating a culture where inclusivity, respect and understanding are at the forefront of its DEIB (Diversity, Equity, Inclusion and Belonging) work.

This anonymous survey is designed to gather insights from service users, and community members, and on how accessible, inclusive, and culturally responsive FTP’s programs and services are. We are inviting clients and community members to share feedback on our programs and services. Your voice matters, and will help us understand who we’re serving well, and who might be missing or underserved. Your input will guide improvements to make our services more inclusive, accessible and equitable for everyone in our growing community.
Informed Consent
We are conducting this survey as part of a research initiative to better understand the needs, challenges, and experiences of our community through a Diversity, Equity, Inclusion, and Belonging (DEIB) lens.

By participating, you acknowledge the following:

  • Voluntary Participation: Your participation is completely voluntary. You may choose to skip questions or withdraw from the conversation or survey at any time.
  • Confidentiality & Anonymity: All responses will remain confidential. No personal identifiers (e.g., names or contact information) will be attached to the data.
  • Use of Information: The information you share will be used only for research and planning purposes by Family Transition Place. Findings may be included in internal reports, grant applications, or public presentations, but your identity will never be disclosed.
  • Emotional Safety: Some questions may bring up feelings or experiences related to barriers and exclusion. You can skip any questions you do not wish to answer and may stop the survey at any time. Please prioritize your emotional well0being as you respond.
  • Consent to Participate: By submitting your survey responses, you are providing your informed consent to participate in this project.
