Disability Access and Inclusion Plan Survey

The Shire is in the process of developing a new Disability Access and Inclusion Plan. Your feedback is important to us to determine goals that the Shire can aim towards to improve the lives of people with a Disability.

We would particularly like to hear from people who identify as having a disability.

Please complete the survey and return to the Shire Office by 4pm Friday 26th February 2021.

1.Do you have a disability?
2.Does your disability effect the way that you access Shire services?
3.Do you attend Shire events such as the Australia Day Breakfast?
4.Do you use Parking facilities at Shire buildings such as the Swimming Pool, Town Hall, Shire Office, and Library?
5.How would you rate the Shire in terms of services available to people with disabilities?
6.Are you able to easily access Shire information?
E.g. Can you easily find out about Shire events and important Shire updates?
7.Do you use the Shire Website?
8.How would you rate the Shire staffs’ skills in assisting people with disabilities?
9.Do you feel that you are able to easily make a complaint to the Shire?
10.Do you feel that people with disabilities living in Gnowangerup are able to have their say and provide feedback to the Shire?
11.Do you feel that people with disabilities are able to access employment with the Shire in the same way that other in the community are able to?
12.Do you have any feedback on our current Disability Access and Inclusion Plan 2017-2021?
