CONSENT FORM

ABOUT THIS SURVEY
This survey is an individually conducted, non-funded research conducted by Valerie Bradley-Holliday. This grounded research study is specifically looking at the Process of Research. The process of research is often taught either from the bottom-up method (deductive research) or the top-down method (inductive research) method. I am looking at developing a theory about the research process based on information gathered from people interested in research. To participate in the survey, you need to:

1. Read the remainder of this document
2. Click on the study link to indicate that you have read this document and consent to participate in the Web survey
3. Complete the Web survey. The survey has 10 questions, and will take approximately 30 minutes to complete.

PARTICIPANTS
People utilizing Social Psychological Network at http://www.socialpsychology.org/expts.htm
and I am seeking input from at least 100 people.

BENEFITS TO SOCIETY
Your participation will provide data that will help increase an understanding of the phenomenon of the research process through five general themes: critical thinking, research process model, intention in research, intuition in research and involvement of stakeholders.

YOUR RIGHTS
In accordance with Walden University’s policy, and as the principal investigator, I assure you that:
1. Your participation in this study is entirely voluntary.
2. You are free to refuse to answer any question at any time.
Refusing or discontinuing participation involves no penalty. To withdraw from the study, simply exit the web browser. Or, if you reach the end of the survey, do not click on the word “Done,” and exit the web browser.
3. Your participation will be anonymous.
4. No personal identifying information is requested. Surveymonkey’s software has a secure server, “no specific users will be tracked,” and only the researcher has access to the survey data.
5. All electronic files after data collection are kept in database form with no personal identifiers. Hard copies will be kept in a secured locked office space with no personal identifiers. All information will be reported only in aggregate form.
6. Upon request, the results of the study can be provided to you in aggregate form after they are formulated.
7. There are no foreseeable risks or discomforts to participating in this study.
8. Please print a copy of this informed consent form and retain for your records.
