Early Foundations Clinics Registration

Thursdays October 8th, 15th, 22nd, and 29th 9am-10am

Please fill out this form to register for the Early Foundations Clinics Thursday mornings from 9am to 10am in North Conference Room 1 at Cheshire Medical Center. There will be directions to get to the room when you arrive at the main entrance.

2.Children are welcome to these classes and childcare will be provided. Will you be bringing children? If so, how many?
3.While we encourage you attend all of the Early Foundations Clinics, it is not mandatory to attend all of them. Please check off the clinics that you plan to attend.(Required.)
