Thank you for taking part in the SWACH Equity and Collaborative Impact Annual Community Needs Survey! Each year, we gather feedback directly from community members to better understand local priorities and challenges. Your input helps drives our policy focus and ensures community voices are at the center of the work we do. You have the choice to remain anonymous or you can input your contact information if you're comfortable. This survey will take roughly five minutes. Thank you for your input.
