This survey is being administered as a part of a research and writing project related to coaching involving diversity, equity, and inclusion dynamics. Your answers will remain anonymous. Please fill out each question to the best of your knowledge after authentic reflection. The terms “diversity, equity, identity, and inclusion” will be used throughout. For the purposes of this inquiry, please refer to the following definitions:
Diversity: Differences in social group identities such as ability, age, gender/gender identity, nationality, race, religion/spirituality, sexual orientation, etc.

Equity: Fair treatment, access, opportunity, and advancement for all people -- based on their individual needs -- while at the same time striving to identify and eliminate barriers that have prevented the full participation of some groups.
Identity: A person's sense of who they are based on the social groups to which they belong e.g., ability, age, gender, national origin, race, religion, socio-economic class, etc.
Inclusion: Active, intentional involvement, recognition, and empowerment of people of all social group identities.
