Welcome to the Nominations Page for the 2016 Lundquist Award for Excellence in Teaching (LAET). For those of you not familiar with this award, it was begun by Samuel Tape, Class of 2005, as his Senior Project. This award recognizes Westside High School teachers who go “above and beyond,” and have made a difference in the lives of their students.

How to proceed?

Please complete this questionnaire.  Please note that, once you begin the process of completing this nomination form, you must complete it, or your answers will not be saved.  Be sure to click on "Done" at the end to submit your nomination.
 
In order for your nomination to be considered, the word count must be at least 250 (all answers combined).  In the past, the nomination forms for teachers who have received the award were roughly 500 to 800 words in length. You may wish to prepare your remarks in a word processor and copy them into the text boxes on the survey.  Please feel free to work in groups.

The person whose nominated teacher is selected as this year's LAET recipient will receive a $50 gift card for Target Stores.
