• The questionnaire will be used in the Practice to help identify where we work well as a team and where there is room for improvement
• All responses are anonymous’ however if you feel you would like to discuss the questionnaire and or your specific responses please talk to your manager.
• This is not a test. There are no “right” or “wrong” answers.
• The questionnaire aims to collect your personal views – please be as honest as possible in answering the questions.
