Primary Health Excellence Awards

Thank you for nominating an organisation or individual in the Primary Health Excellence Awards.

Eligibility
To be eligible for the Western NSW Primary Health Excellence Awards, nominees must meet the following criteria:

• Open to individuals, teams and organisations delivering primary health care services across Western and Far West NSW.
• Nominees may be based within the region or delivering services that predominantly benefit communities across Western and Far West NSW.
• The work, service or achievement being nominated must have been delivered or demonstrated within the 2025–26 financial year.
• Nominations are open to a broad range of primary health care professions and service providers, including (but not limited to) general practice, allied health, aged care, pharmacy, nursing and Aboriginal health services.
• Individuals, teams and organisations can be nominated by themselves or by others.
• Nominees may be submitted in more than one award category, provided they meet the criteria for each category.
• Organisations that are directly involved in delivering or administering the awards program (including sponsors or judging panel members) are not eligible to enter.
