In consideration of my voluntary participation with Community Resource Center, I hereby agree to the following: 
  • I hereby release, waive, indemnify and hold harmless Community Resource Center, its directors, officers, employees and volunteers from any loss, liability, and damage due to my voluntary participation with Community Resource Center.
  • I hereby assume full responsibility for the risk of bodily injury, death or property damage.
  • I further agree that the foregoing release and waiver of liability and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the state of California, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
  • I have read and voluntarily sign the release and waiver of liability and indemnity agreement, and further agree that no oral representations, statements or inducements, apart from the foregoing written agreement, have been made.

Photo/Video Release 

I give my permission and consent to Community Resource Center for the use of any photographs or video of me and/or my child’s participation at Community Resource Center events/programs/locations for publicity purposes, without compensation. I further understand that media may be present at CRC events/programs/locations and that I or my child may be photographed or filmed while participating in the program/volunteering.

If I choose not to have myself and/or my child photographed or filmed, it is my responsibility to inform the photographer.
