Substance Use and the Workplace: Employer Survey
Purpose and Confidentiality statement:
Purpose:
The purpose of this survey is to hear from people who work at local businesses about their experiences related to Substance Use Disorder* and the Workplace.
Information from this survey will be used to develop training and workplace policies that support people with substance use disorder, and provide a safe and healthy work environment for all staff.
*Substance use disorder, or addiction, is defined by the National Institute on Drug Abuse (NIDA) as a “chronic, relapsing brain disease that is characterized by compulsive drug seeking and use, despite harmful consequences.” Substance use disorder is a brain disease because drugs can cause long-lasting changes to the way the brain functions.
Confidentiality:
Your participation in this survey is 100% confidential. This survey will not inquire about your personal identity or place of work. If you choose to provide information that could be linked to your identity or place of work, the information will be removed from the overall survey results to ensure confidentiality.
