General Dentist Esthetic Preferences

* 1. Introduction:
You are being asked to take part in a research study conducted by Dr. Abanob Sarkis under the supervision of Dr. Daniel Rinchuse at the Seton Hill University Center for Orthodontics. Please read the following form carefully and ask any questions that you may have before deciding whether to participate in this study.

Procedure:
This study involves one survey that takes 3 to 5 minutes of your time. You must be 18 years or older to participate.

Risks/Benefits:
There are no risks. There are no direct benefits to you from participation, but this may provide a better understanding of esthetic preferences.

Confidentiality:
Your identity will not be exposed at any point during this process and privacy will remain intact. Your answers will be completely anonymous.

Voluntary Participation:
Participation in this study is voluntary. If you do not want to be in this study, you do not have to participate. Even if you decide to participate, you are free to withdraw from participation at any time without penalty.
