Sustainability Planning Survey |

The Sustainability Planning Project aims to support people with Intellectual and Developmental Disabilities (I/DD) as their natural caregivers age and when they can no longer provide support to their person living with I/DD.
We invite you to participate in this survey to help identify challenges, opportunities, and necessary resources for successful long-term planning across the lifespan. You have been invited to share your experiences because you or a person(s) you support has I/DD and the person(s) with I/DD lives in Maryland. Your responses will inform policy, programs and planning for sustainable/long term, person-centered support and services.
Your completion and submission of your survey responses is implied consent to use your responses in aggregate form. All responses will be confidential and you will not be identified in any reports we publish.
This project is made possible through a grant from the Developmental Disabilities Administration to SDAN in collaboration with Collaborative Strategies.
