Family SERVICE VIP Dinner Survey

Hello!  You are receiving this survey because you either attended or supported the Family Service of Roanoke Valley 2019 Celebrity Tip Off event.  This year, we are re-branding it to the Family SERVICE VIP Dinner.  The dinner was to occur in May, but of course due to COVID-19, we postponed it to Thursday, October 8th.  We are planning another great event featuring local celebrities and a silent auction.  If the event takes place in person, it will be at the Hidden Valley Country Club from 5:00-8:00PM.  We plan to have most of the event take place outside on the patio to allow for plenty of social distancing and will encourage the wearing of masks (except while eating).  Social distancing will also take place inside; the number of seats per table will be greatly reduced, etc.  

While we would love to hold an in-person event, we want to make sure we are moving in a direction that is safe and comfortable for everyone.  

The survey includes only 4 questions and should take 5 minutes or less to complete.  Your responses will help us determine whether we hold the event in-person or virtually.  We appreciate your time and effort in helping us to make this important decision.  
1.I would be comfortable attending the event in-person as described above.
2.I would be comfortable wearing a mask at the event (except while eating).  
3.What local celebrity would you like to see support the event as a server?  
4.If the event takes place virtually (via the Zoom Platform), on the same date and at the same time (Thursday, October 8th, starting at 5PM and ending no later than 6PM), I would definitely be interested in participating.
