Dear Pope County Community Member,

You are invited to participate in a survey concerning substance misuse and addiction issues in Pope County, IL. The purpose of this survey is to assess the knowledge, attitudes, and beliefs regarding substance use disorders (SUD) and opioid use disorders (OUD). Also, this survey aims to collect information about the perception of Pope County’s prevention, treatment, and recovery services in relation to SUD and OUD. This survey is funded by the Health Resources and Services Administration, and is being conducted by the Center for Rural Health and Social Service Development at Southern Illinois University School of Medicine.

The survey will take approximately 10 to 15 minutes to complete. If you choose to participate, your responses will remain anonymous and will be combined with those of approximately 350 other participating community members. Your responses will be used to help identify service gaps and develop recommendations for addressing these needs in the Pope County community. Participation in this survey is completely voluntary and declining to participate or stopping the survey at any point will not result in any consequences including loss of services or benefits. There are no known risks or benefits associated with participation in this survey.  The survey data will be stored on a password-protected computer and a secure server in a locked office.
