The Project
CADDRA - Canadian ADHD Resource Alliance is developing an education program to build knowledge, skills, and confidence about ADHD and substance use among clinicians and other service providers.

Are you:
  1. A clinician who currently provides ADHD care for individuals (who may or may not also have a history of substance use / substance use disorders)?
  2. A clinician or other service provider whose primary focus is substance use care?
  3. An individual with lived or living experience of having both ADHD and substance use/substance use disorder?
  4. A family member with experience navigating care of a loved one with both ADHD and substance use/substance use disorder?

If so, we want to hear from you!

The Invitation
You’re invited to complete a short survey and help us design our program. Please proceed to the next section and answer a short (5-10 minute) survey about your experiences providing, receiving, or facilitating care. The information collected will be used for the purposes of program planning. We will not contact you unless you provide us with your contact information and wish to be involved in an ongoing way.
