Healthy Blair County Coalition - Community Health Needs Assessment
Blair County, like most counties in Pennsylvania, is taking an extended look at ways to better utilize our assets to impact community needs. As part of the effort to build a better community, the Healthy Blair County Coalition is conducting a community health needs assessment. Surveys are being sent to a variety of groups but we are asking for your help in completing the Business Asset Survey to learn more about the strengths and assets we have available. We are contacting you because we highly value your opinion, and we believe your insights will help improve all aspects of a healthy Blair County.
Beginning in 2012, all hospitals are required by the Affordable Care Act to conduct a community health needs assessment and to develop an intervention plan to meet those community health needs. However, members of the Steering Committee view this as an opportunity to assess and hopefully impact all aspects of a “healthy Blair County” so we encourage participants from all segments of the community to participate. A healthy Blair County means impacting social, economic, emotional, and physical needs of residents and the community itself.
Please complete the survey no later than August 31, 2012. The survey only takes a few minutes to complete and the questions ask for specific responses that will be compiled into a database for purposes of ascertaining volume and trends. Your individual responses will be kept confidential because we are not tracking who completed the survey. This information will not be used to solicit your business or to encourage you to participate in projects or programs undertaken by any of the participating organizations. However, your participation will help ensure that this is a successful effort and we also invite you to join the Healthy Blair County Coalition.
If you have questions about this survey, please contact Coleen Heim, Director of the Healthy Blair County Coalition at email@example.com or 944-0884 ext. 305. For more information on the coalition or how to become involved, please visit our website at www.healthyblaircountycoalition.org.
