Buckelew Programs - Speaker Series Questionnaire

Buckelew Programs is creating a Speaker Series to provide information, expertise and positive strategies to families of individuals who are living – and aging - with mental illness, as well as other mental health and substance abuse issues. The goal is to create a safe place to explore topics related to managing health and wellness, long term care and housing,  financial and estate planning,  understanding Social Security benefits, and more.  It is our hope that information provided at these talks will help you plan for the future, avoid costly and irreversible mistakes, while allowing you and your family to minimize worry and  live healthy and productive lives for as long as possible.

To ensure we are bringing you the most relevant speakers and topics, we have created the following questionnaire. Please take a moment to complete the four questions to give us your feedback as to programs you would be like us to consider.
