Introduction and Directions
Hello New York State School Food Authority!
Thank you for opting in to the Drive for 25 Grant Farm to School Grant for School Year 2016-2107. We are excited to work with you over the next year to purchase qualified New York State grown or processed foods for your school meal programs.
In order to enroll you in the reimbursement procedure for the grant, you are required to submit information about your past purchasing history of New York State products, promotion of NYS products and where NYS foods were served in your schools.
Data from this baseline report will help us make changes to the program to better serve your needs and help us measure the success of the Drive for 25 Farm to School Grant.
The survey should take you between 15 minutes to 1 hour to complete.
REMINDER: You will not be able to submit a reimbursement claim until we receive your complete baseline data report. We recognize that record keeping is imperfect and ask that you do your best to provide accurate information. This data will be essential for reporting program impacts and helping to secure continued funding.
