Thank you for taking the time to assist us in our program planning by completing this online survey. 

As you may know, Kawartha North FHT has been fundraising for a number of years to operate our walk-in clinics in Fenelon Falls and Bobcaygeon.  Whilst we receive funding from the Ministry of Health to provide primary health care services to our patients, we receive NO FUNDS to operate the walk-in clinics that serve our communities.

The funds raised in the communities have been used to pay our Nurse Practitioners and reception staff.  Hiring staff comes with the added responsibility of requiring reserves to guarantee wages for at least a minimum amount of time.  We are hoping to expand and we would like input from our donors, volunteers and clinic patients on some options for expansion.
 
The survey should take between 5-10 minutes to complete and all responses are completely anonymous.
