NSW Ambulance Health Provider Feedback

GPs are recognised as an important stakeholder by NSW Ambulance and the PHN.  Your feedback will greatly assist with the monitoring and evaluation of this on-going 'model of care'.

1.I believe that the patient was given appropriate clinical advice for the management of their condition by Ambulance Paramedics
2.The patient did not experience an adverse outcome and was able to be appropriately managed by me 
3.The patient benefited from non-ED care 
4.Appropriate clinical handover was provided by Paramedics 
5.The patient should have been referred to the Emergency Department 
6.Do you have any other comments about how we can improve our clinical handover to GPs
