Providing an email address is voluntary and allows us to follow up with you with your consent. The feedback itself remains detached from your identity as responses will be aggregated, and no personal identifying information is linked to your answers.
Your anonymised feedback and data may be used to support grant applications and to promote PoTS UK services on our website and social media channels.
Your honest feedback is so important as it will enable us to adapt and develop the services we are offering in order for us to give you the support you find most useful.
This survey should take about 10 minutes of your time (your answers will be saved if you are unable to complete this in one go), and should be completed by people with PoTS, suspected/self-diagnosed PoTS, or parents and carers of someone with PoTS.
PoTS UK are planning ahead and want to learn more about what YOU want and need, so please feel free to contribute as much information as you think necessary.
