Title of Study: K-12 Student Success and Conduct in the Digital Classroom: Investigating the Role of Virtual Learning Environments

You are invited to participate in a research study conducted by Kelley Schmidt, a[GRL1] Doctoral candidate in the Department of Leadership Studies at Converse University in Spartanburg, SC. You must be at least 18 years of age to participate in this study. Please read this form and ask any questions you may have before agreeing to be in the study.

PURPOSE OF STUDY: The purpose of this study is to research the success of students who participate in virtual learning environments. Data collected from this study will be used for a doctoral dissertation, [GRL2] and possibly published or presented by the researcher.

PROCEDURES: If you agree to take part in this study, you will be asked to complete a survey that will take about 20 minutes. You will not be asked for your name, and all answers will be strictly anonymous[GRL3] .

VOLUNTARY PARTICIPATION: Participation in this study is voluntary. You may stop participating at any time. There is no penalty for not participating.

RISKS and BENEFITS: There are no anticipated risks or discomforts to your participation in this study other than those encountered in daily life. [GRL4] If you feel discomfort, you are advised to seek support from your medical practitioner or a professional counselor to ensure your well-being and prevent any unintended accidental harm.

Although you may not directly benefit from this research, by participating in this study, your responses will help others in the future. The results could contribute to programs with virtual learning environments and help to shape the methods of instruction.

CONFIDENTIALITY: Your participation in this survey is anonymous; you will not be asked for your name. Survey data will be kept on a password-protected computer with access only to the researcher and faculty supervisor. After the study, data will be kept secure for a minimum of [GRL5] three years, and then it will be permanently deleted. The survey will be administered through SurveyMonkey.
