2018/2019 Coaches Evaluation Survey |
INTRODUCTION
Welcome to the 2018/2019 Coaches Feedback Survey
This electronic format was chosen due to it's security, confidentiality and ease of use. Additionally, the survey responses can be easily tabulated to provide a much more valuable document for coach evaluation and feedback.
WHAT THE COACH WILL SEE: Once the surveys have been tabulated, the coach may be be provided with a summary of the responses. Example: "88% of those responding agree the practices were effective".
WHAT THE COACH WILL NOT SEE: The coach will not be provided with information in regards to WHO filled out the survey, thus allowing the coach to NOT identify the respondent - thus, the survey is completely confidential. However, as we do NOT want to edit or 'black out' words or comments within a survey, coaches WILL BE provided with direct comments from the survey - which will include individual identifiers if YOU place them there within the body of your text responses.
IMPORTANT: You must provide the following information on the survey where requested - failure to provide the information will result in the survey being discarded:
1) NAME: The name of the person(s) completing the survey. The name(s) listed here must correspond to either a) player on the team, or b) parent(s) or guardian(s) of the player on the team. This step ensures the correct people are evaluating correct the coach associated with that team.
2) PLAYER NAME: The name of the player needs to be identified in order to match the survey with the correct coach
3) E-MAIL: The e-mail address you used to register your child in Edmonton Ringette. The e-mail must match up with the one on file at Edmonton Ringette otherwise the survey will be discarded. Note: there is an area to explain 'why', if there is a difference in the e-mail address.
Upon completion of the survey, the PLAYER'S NAME will be entered into a draw for ONE FREE REGISTRATION for the 2019/2020 Ringette season.The PLAYER'S NAME will be entered into the draw only one time. THIS YEAR WE WILL BE DRAWING 1 NAME.
Partially completed surveys will not be accepted.
The survey takes approximately 20 minutes. You must complete the survey in one sitting as you will not be allowed to return to the survey at a later date or time.
Both PLAYER and PARENT(s) can fill out a SINGLE survey on a coach as long as they were either 1) a player on that coach's specific team or 2) a parent of a child on that coach's specific team.
If you have more than one child in Edmonton Ringette and they each played on different teams, please fill out a separate survey for each coach.
