Informed Consent

Dear Participant,

My name is Dennis Apker, and I am a Ph.D. candidate in the Doctoral Program in Clinical Psychology at the University of Detroit Mercy. I have asked you to agree to be a volunteer in some research I plan to conduct.  Before I can accept your consent, I want to make known to you the following information pertaining to the project.

I am seeking approximately 120 volunteers to complete some surveys regarding their membership in different types of communities and measures of wellbeing, thinking patterns, and attitudes. The purpose of this study is to investigate the characteristics of members of different kinds of communities. The completion of these surveys will likely take approximately 30 minutes.

There are no expected risks entailed in your participation with this research. Some survey questions may be experienced as mildly intrusive, but it is not expected that these questions will result in any substantial emotional discomfort. Regarding potential benefits of participating in this research, it is possible that some questions may result in you learning about yourself if they are questions you have not yet been asked or thought about.

Furthermore, an incentive for your participation is an opportunity to enter the drawing for a $20 Amazon gift card. Your participation in this drawing is entirely optional. At the end of these questionnaires, you will be directed to a different survey for the sole purpose of entering your email, which enters you in the raffle. Because you'll be directed to an entirely different page to enter your email, there will be no way of connecting your identifying information (i.e., your email) to your responses.

Your confidentiality is a priority in this process.  Your responses are stored on my personal SurveyMonkey account, which is password protected and only accessible to me. The confidentiality of the records will be maintained unless the law requires disclosure.

The University of Detroit Mercy Institutional Review Board has approved this research project and the forms involved in the study. If you consent to be a volunteer in this research project, you are nonetheless free to withdraw your consent and discontinue participation at any time without prejudice to you. Eligibility for the raffle, however, requires that you complete the surveys. You should also understand that the investigator has the right to withdraw you from the research project at any time.
