To complete this form the school superintendent (or designee) will need to have access to the following information/documentation:

Total Enrollment Percentages in Baseline CS Courses* (percentages based on total student population)
Growth in Enrollment Percentages in Baseline CS Courses* (based on percentage increase from 2016-17 to 2017-18)
Number of CS Courses with Students Enrolled (documentation may be a letter on school letterhead signed by the superintendent confirming the course names, course code numbers, number of students enrolled in each section)
Number of Teachers with CS Endorsement (AELS Case ID#s for each teacher will be required and is accessible here https://adeaels.arkansas.gov/AelsWeb/Search.aspx)
Documentation of CS Education Week Events (documentation may be a letter on school letterhead signed by the superintendent confirming the event and level of participation)
Documentation of Promotional Events (documentation may be a letter on school letterhead signed by the superintendent confirming the promotional events that took place)
