Important information to read prior to completing the form.

This project has been designed to evaluate how taking and applying our Viridian Triple Action Hair System (Hair Support Complex Capsules, Hair Growth Liquid and Hair Restore Serum) for 12 weeks can benefit your head hair.

We understand that lack of hair growth and hair loss can have a big impact on overall health and confidence. With this in mind, we are looking for individuals with accelerated hair loss, increased hair shedding and poor hair growth to participate in our trial. It will involve answering questionnaires at the beginning, after 8 weeks, and again at the end of the trial, and providing photographs of your hair as required at each stage.

To be part of the trial, you would be required to:

· Complete the initial questionnaire on the next page which will include us asking you for details about your health and medications as well as the condition of your hair and scalp (Takes 10-20 minutes for the questions plus time to upload your photos)

· Start the trial within 10 days of receiving your Viridian System products and continue for 12 weeks uninterrupted

· Completion of interim 8 week, and end of project 12-week questionnaires via secure link (Takes 5-10 minutes for the questions plus time to upload your photos)

· Photograph certain parts of you hair and scalp at the start, interim (8 week), and end of the project (12 week)

· Take 2 capsules of the Hair Support Complex with food daily

· Take 5ml of the Hair Growth Liquid with food daily

· Apply the Hair Restore Serum at least 2-3 times a week and leave on for at least one hour before washing your hair

· To adhere to your usual style of eating and lifestyle


For safety reasons we are unable to consider you if you:

· Are trying to conceive, or are currently pregnant or breast-feeding

· Have allergies to nuts or seeds

· Are taking immunosuppressants, undergoing cancer treatment

· Are using oral or topical products already or within the last 6 months for hair growth/loss

· Are a Viridian Nutrition employee

If any of these apply to you, unfortunately, we won’t be able to include you in the trial

It may be possible to join the trial if you are taking medication but we need to check that the Viridian system is suitable alongside so you will be asked to give details of all medications you are taking.

Every time you take pictures, please take under similar lighting conditions, from the same distance and in the same position. Please leave you hair loose and ideally comb or brush beforehand. This photo is too dark.
Make sure the photo does not include your face or cover the hairline or focal point.
