Mental Health and Addictions Pilot Project Survey – Health and Social Services Professional

The Niagara Ontario Health Team - Équipe Santé Ontario Niagara (NOHT-ÉSON) is undertaking a project to understand better the current gaps in accessing community services and develop system solutions to address the emergency departments (ED) and urgent care centres (UCC) being the first points of contact for mental health and addictions-related (MHA) care.

As part of this initiative, we are seeking to engage with health and social service professionals to understand why 18 to 25-year-olds from Fort Erie have visited an ED or a UCC.

Preliminary data has indicated that approximately 20% of Fort Erie residents from this age group have visited an ED or a UCC as the first point of contact for MHA-related care in the last two years.
1.Which of the following best describes your current role?
2.Are you a health or social services professional representative in Fort Erie?
3.To the best of your knowledge, has your patient, client, or a family caregiver of the patient accessed any MHA-related care in any ED or UCC in Niagara in the last two years?
4.If yes, to the best of your knowledge, was this the patient’s first visit to an ED or UCC for MHA-related care, or had they visited prior?
5.If you answered YES to the two questions above, which ED or UCC in Niagara did they access for MHA-related care in the last two years? (Select all that apply.)
6. To the best of your knowledge, what time of the day did they visit an ED or a UCC?
7.Tell us why the person may have decided to visit an ED or a UCC for MHA-related care versus seeking help elsewhere in the community.
8.To the best of your knowledge, did the person seek help from any community MHA-related services in Niagara prior to the ED/UCC visit?
9.Do you know if the person has a primary care doctor or nurse practitioner? (If you are the primary care provider, please proceed to the next question.)
10.On a scale of 1 to 10, 1 being not satisfactory and 10 being satisfactory, how would you rate the availability of MHA-related resources and services in the community?
