Lincoln Park survey

The Creating Healthy Communities (CHC) Program of Allen County Public Health is committed to improving access to physical activity for people of all abilities. CHC has partnered with the City of Lima to make improvements at Lincoln Park. We would appreciate your participation in this brief survey to evaluate the project improvements. Thank you!
