Customer Satisfaction Survey for Community Employment Services (CES)

This is the online version of the CODI Satisfaction Survey CES. If you have already filled out the paper version, please do not use this online form.

The purpose of this survey is to gauge how satisfied customers are with CODI Community Employment Services. In order to do this we need to know your opinions.  You do not have to identify yourself but you may if you wish.
