Instructions 

Before you begin this application, please work with your planning committee to develop your educational plan.  This application will require you to describe the following information:

- Your event details

- List of planners and faculty

- Activity format and rationale for that format

- Practice Gap Analysis (what is occurring now and what educational needs are you meeting?)

- How you determined your Practice Gap (ex. survey of members, scholarly article, etc)

- Educational Outcomes
Applications and attachments must be submitted at least 4 weeks prior to the distribution of publicity about an educational activity for review of compliance with ACCME standards.  Please contact sdesantis@shm.org about your proposed CME activity before completing and submitting an application for the first time.
 
Applicants are expected to have read SHM’s CME Guidelines prior to the preparation of this application.  This planning form collects all information necessary to plan and develop your proposed CME activity.  Completion of all sections of this form is necessary to meet ACCME accreditation requirements.  Please note that no CME activity will be approved retroactively.
