Learning from loved ones' experiences of end of life care

1.Thinking back, when did you first realise that your relative was becoming seriously unwell or approaching the end of their life? Did you feel that was early enough to discuss their care and support?
2.Did your relative have the opportunity to talk about what mattered to them?
3.Were your relative's wishes recorded and shared with the people who needed to know? (Advanced care plan, preferred place of care)
4.When your relative became more unwell, did the care they received reflect their wishes?
5.If their wishes could not be followed, what stopped that from happening?
6.Can you tell us what happened? (E.g. did they receive the right support at home? Was the GP involved? Were community/district nurses involved? Was hospice support offered? Was palliative care involved? Did anyone anticipate what might happen next? Did the family know who to contact if things deteriorated?)
