Lutz Preparatory 2019-20 Kindergarten through Eighth Grade Parents/Guardians,

Please complete the following REQUIRED 2019-20 School Year - First Day of School Attendance Census. If you have multiple students attending the K-8 school you must complete ONE census form PER student. Please note, this is NOT for preschool students attending Lutz Pre-Prep, only K-8 students enrolled in the public charter school.

Deadline for submission is Friday, July 12, 2019, at midnight.

You will receive a confirmation screen at the end of this form stating "THIS PAGE CONFIRMS RECEIPT OF YOUR FIRST DAY OF SCHOOL ATTENDANCE CENSUS" with additional details and ending with "GO BOLTS". If you reach that confirmation screen, your submission has been uploaded to the database. Please do not contact the school asking for a second confirmation of receipt. We will contact you if your student is missing any enrollment requirements.
