Between 2016-2018, you should have received an Oral Health Story Kit from Healthy Teeth Healthy Children, a program of the PA Chapter of the American Academy of Pediatrics, to be used as a resource around the theme of good oral health with the children in your community. This follow-up survey will help determine the usefulness of the kit and will help us improve community resources like this in the future. Please complete the following brief (<5 minute) survey.
