Stakeholder's Survey for Residential Consumers for the Division of Developmental Disability (DDD)

To our Stakeholders:

This is the online version of the CODI Stakeholders Survey. If you have already filled out the paper version, please do not use this online form.

The purpose of this survey is to gauge how satisfied Stakeholders are with CODI services.  In order to do this we need to know your opinions.  You do not have to identify yourself but you may if you wish.
