Thank you for taking the time to complete our survey for the Colorado Respite Coalition, a program of Easterseals Colorado. The Primary/Family Caregiver served by the voucher or respite grant should answer these questions. Your feedback is very important and helps to determine funding.
Before beginning, please note the following:
"Primary/Family Caregiver" refers to the family member, friend or primary person caring for the person with extensive care needs at home. This person provides 40 or more hours a week of unpaid caregiving. For example: If Mary is Bobby's grandmother and takes care of him the majority of the time, she is the primary/family caregiver.
"Individual receiving care/Care recipient" refers to the person with extensive care needs that is receiving care.
