Transition - Moving from pediatric to adult CHD care

Canadian Congenital Heart Alliance has created a survey with the purpose of gathering feedback from teens, young adults and adults living with CHD, and their families about their experiences with Transition. We are looking for input from those currently in the process of Transition, those that are now in adult CHD care or those that have children that will be transitioning in the future.

This information will be used to create educational resources and materials as well as provide support for families transitioning to adult care.  You can choose to keep your responses anonymous or provide your contact info so we can include in educational materials and resources and share on social media.
