Welcome to the Sport for Good Annual Membership Profile. If you completed the 2021 survey (full list here) the YOU DO NOT NEED TO COMPLETE this survey as your information is still valid.

By contributing information to this form, you are helping to build a better understanding of our city and national coalition as well as the wider field. Information will be aggregated by city and will be shared with participating organization’s for their own advocacy, fundraising, or other purposes. We will only share aggregated data, not that of individual organizations, so your information will remain anonymous.

Once an organization is registered, all staff will be offered access to the online Sport for Good network.

Please Note:
Select “Youth-Serving Organization” if you are signing up for an organizational membership on behalf of your institution/ program/ school/ agency/ etc. that works directly with youth participants. 

Select "Non-Youth serving Organization" if you work for an organization that does not do direct service work with youth.
