To our valued JFM Patients:  We at JFM are possibly looking at going to a different pricing structure and are interested in your feedback.  Please answer the following questions and submit this back to JFM.  All responses are anonymous.  Please submit your answers no later than January 31, 2023. Question Title

* 1. Would you continue your care at JFM if, in addition to current insurance billing, there was an annual fee per adult that covered:

1) Monthly classes on important health topics
2) Completion of forms and letters outside of an office visit, free of charge (FMLA, sports/work/school physicals, daycare etc.)
3) Continued access to our patient portal for results and conversations with your provider (will be a benefit for paying patients only)
4) Opportunities for telephone and/or portal medical advice without an in person or virtual office visit (up to provider discretion) that would have otherwise generated a co-pay
5) Help in our office to work to decrease medication costs

Would you continue your care at JFM if we implemented a model called Direct Primary Care where a monthly membership charge of around $100 per adult allowed you to be seen at your convenience and have care routinely without an appointment? This model does not allow JFM to bill any private insurance, Medicaid or Medicare.
